Payment Agreement

Payment Option Agreement
- You Understand that you have chosen a payment option for our program and that after paying the first amount, there will be a second amount due 4 weeks after the start date of the program.
TERM OF AGREEMENT
- The term of this Agreement (the "Term") will begin on the date of this Agreement and will remain in full force and effect until the completion of the Services, subject to earlier termination as provided in this Agreement.
REIMBURSEMENT OF EXPENSES
- There will be no reimbursement after week 2 of the program. If you choose to opt out of the program, all paid money will be forfeited. After week 4 the remaining balance will be due, and there will be no refunds.
CONFIDENTIALITY
- Confidential information (the "Confidential Information") refers to any data or information relating to the Client, whether business or personal, which would reasonably be considered to be private or proprietary to the Client and that is not generally known and where the release of that Confidential Information could reasonably be expected to cause harm to the Client.
- Sandi agrees that they will not disclose, divulge, reveal, report or use, for any purpose, any Confidential Information which Sandi has obtained, except as authorized by the Client or as required by law. The obligations of confidentiality will apply during the Term and will survive indefinitely upon termination of this Agreement.
- All written and oral information and material disclosed or provided by the Client to Sandi under this Agreement is Confidential Information regardless of whether it was provided before or after the date of this Agreement or how it was provided to Sandi.
I Agree to the above terms and authorize Sandi Santiago (The Liminal Soul Healing) To charge my card below, 4 weeks after the program start date.
I agree to pay for this purchase in accordance with the issuing bank cardholder agreement.
